Nixon-Smiley CISD is hosting a FREE COVID vaccine clinic for the public, students, and staff.
All 3 COVID vaccines will be available: Pfizer for ages 12+, Moderna, and Johnson & Johnson for ages 18+
1st Dose: September 9, 2021, 10:00am – 3:00pm
2nd Dose: September 30, 2021, 10:00am – 3:00pm
Johnson & Johnson is one dose/one appointment.

All of these forms and the vaccine EUA fact sheets are attached to the registration link. The highlighted forms are the ones DSHS needs in advance for ages 12-17 if the parent will not be present. They can be emailed to Region8COVID-19@dshs.texas.gov, or brought to Lisa Jackson, RN MSN, District Nurse at the high school campus ahead of the event. (Note – some of these forms are subject to change due to the FDA EUA and Approval changes, but the most recent versions will be provided as they are updated)
